Eyeglass Lab inc - Excepts all insurance claims as an Out-Of-Network Suppier once you have made and completed your online purchase a computerize reciept will be generated and sent to your email address we have on file for you can then summit this reciept along with your claim form to your insurance company for direct reimburstment.
Most insurance companys have an out-of-network program set up for its members but some do not so we reccomend that each customer contact their insurance company to make sure they can go out-of-neywork for their vision products.
We accept Flex Spending you can use Flexible Spending Account to make online lens purchases and get itemize reciept to summit for direct reimburstment.
Eyeglass Lab inc can supply some out-of-network insurance claim forms.
